  This form is required for all applications for independent vaccinator authorisation (either first time or renewal applications).
• Independent Vaccinator Clinical competency assessment form (370kb pdf)
  This form is only required if you are reapplying for authorisation renewal and you require a peer-reviewed self-assessment.

  This form is used if your immunisation programme is not currently approved and requires programme approval.
